Difficulty: Hard
Hint: This is one of the hardest to unlock. I will give some points on how to get it.
Start a game on Breakdown Level 2 or higher then go to a base. Don’t get any Medicine to your base or it will take longer to unlock. Don’t construct a Medical Area. Try to get sick from a Bloater, then you can play until the sickness spread and a survivor with critically ill status goes missing. Keep in check if you have 2 or more ally’s missions active, you can’t get the rescue mission and then they could die. After rescuing the critically ill survivor, the mission to kill him will unlock later (can also unlock soon after you turned him in).
To get this soon you need to go offline, then load your game. If there is a missing survivor, get the missions done and rescue him. If not, close the game, advance a day in your system clock and them load your game again. Repeat until you see a missing survivor. This is the method that I used and took a month in day advancing to unlock.
